It was required and checked at Cebu Airport when I entered Philippines on 15th of January 2023.
I cannot find any information that this procedure was changed or cancelled.

Last time I was abroad was last October (Singapore) and had to fill it out. It's a very simple form, though. Even if it were still compulsory, it's minimum hassle.
